Do you have to use sheets that will fit a 20” mattress? (Not sure if there is such a thing.)

I bought a set of sheets for a full size bed. I clipped and trimmed the bottom fitted sheet. Now I will just buy flat sheets for a full size bed to eliminate fabric waste.

Can you adjust firmness of mattress?

Yes you can. The 'firmness' is a function of how much air you pump into the mattress using the built in electric pump. If you put too much air in and the mattress is too firm, you can release air using the deflate valve. This mattress is Great!

Does it come without built-in pillow? Can pillow be removed?

The built in pillow is just a raised area on one end and not removable. We just laid a regular pillow on top. Super nice mattress, very comfortable and easy to set up and take down too.

Is it very heavy to move once inflated?

No not really. Once time we kept it up all day and just moved it around. Another time we took it down each morning and re inflated it. It does tatke up space so depends on where you're putting it. It was a nice comfy bed I like the ability to inflate it to what level you like.
