Fixed my problem!
I replaced this canister flush valve, part #1117210, on my Kohler Wellworth (aka Highline) Classic, Class 5, left handle (LH), 2 piece, single flush toilet, tank #4436, model #3513. The part on top of the flush valve where the water hose connects was gone and water was spewing under the tank lid when I flushed. I hunted on Kohler's website for this part, which was tricky to find depending on my word search. I finally found it and some other websites corroborated the part number. I installed just the top piece (canister part). The water tube connected tightly and stopped spewing tank. I replaced this along with fill valve GP1083167, which solved my leaking toilet issue.

Assembly works, but I later found I only need to replace one rubber gasket
This replacement looks identical to the original and it worked. But after I spent the time replacing it and I had examined the old one out of the toilet, I realized the entire assembly did not need to be replaced. My old assembly was leaking where the top moving part drops down to the lower connecting part. There is a round, red, rubber gasket under the top part and it fits in a groove. It had stretched out over the past 10 years and was allowing water to leak. I think if I would replaced it only, it would have fixed the problem.

All's Well Now! Easy-ish to install. PAIN to remove!
HELLS BELLS! Like others have stated, this thing is a total PITA to take a part. Especially if you don't have a garage full of tools. I was barely able to get the bottom nut off by half butting it with a vice grip that just barely did the job. B-A-R-E-L-Y! Would definitely suggest making sure you have a plumbers wrench or something of the sorts.Other than that, it was relatively easy to install the new parts. I'm leaning towards all that was needed was a new seal, but we opted just to replace everything since the hose from the flush valve to the fill valve looked like it was there since the 1960's. We got all new innards for my mothers toilet and while the fill valve kit came with instructions, this did not. This came in a plastic bag. A plastic bag that now that I think of it, is EXACTLY like the plastic bags I've received from Amazon before that came from the warehouse as renewed or an open package, when this was supposed to be BRAND NEW. Definitely would have thought this would have come in Kohler packaging, if not a box, like the flush kit did. Only issues I had with the install was the typical lack of space that 99.99999% of toilets provide around them to do anything and stuff like that.I didn't have any issues with the bottom rubber part and the screws, like I read others seemed to have. Did kinda feel like the top part had a little extra pull to it, while the sides were snug. Not sure if that was manufacturing and/or a "me" thing.Toilet is no longer running and all's well!

Received used dusty and ugghh
Received used and dusty, looks like it was a return item because of the scratched black plastic on the canister. No other words but ugghh

Perfect fit.
This is Kohler specific, which is a catch 22 kinda thing. The design and function is great, but a royal pain when you need to change it. Unlike a simple flapper design, installing this requires complete removal of the tank and replacement of all the seals and gaskets. Removal of the large nut holding the valve on is not easy. I found it best to just cut the old seal off, this makes it easier to access the nut. Once I knew what I was doing the 2nd one took about 30 min. They did make one improvement by putting a nipple on top of the valve for the bowl filler hose as opposed to the old cone thing the original had.

Genuine Kohler Part but Perhaps You Have No Need to Replace this Part
My Kohler K4436 toilet was internally draining down its water storage tank over long periods of no use -- say a couple of days. I jumped to the conclusion that I had to replace the entire flush valve in order to replace its internal seal. Well, live and learn! The internal seal is easily replaced without removing the entire flush valve. If your problem is similar to mine, just buy the Kohler GP1059291 (yellow) gasket -- available on Amazon. On the back of the package are instructions giving the simple 3 or 4 steps needed to replace the gasket by just removing the top cover from the water storage tank. Also, even if in your circumstance you do need to replace this flush valve, buy the yellow GP1059291 gasket as well. The package notes that it is an improved material that fights leaks longer than the standard red gasket provided in the flush valve kit.Replacing the flush valve is easy (instructions are included), but do be aware that you need to remove a 62 mm (2-7/16") nylon hex nut from the underside of the storage tank. Thus, have a pair of appropriately sized Channelok (sp?) pliers on hand to complete this step. Note that the kit does include an new nylon nut for this location.
