Q: Do all GTX100's come with the foot pedal in the box (the version normally priced at $499)?

Yes they are supposed to mine did you need it to be able to do the looping thing it’s a great amp !

Q: Gtx 100 has headphone out and 2 xlr outs, right and left for stereo. how big of an amp would be needed to run the stereo option?

Typically the XLR outs are for a PA system to guve you stereo effects, but you can always pre or post out to another amp of any size

Q: Does the GTX-100 have a mic input?

No. It has an AUX input (which could be use for a mic with the proper plug, I suppose) and an instrument input only. But no dedicated mic input. It also has headphone out and 2 XLR outs, right and left for stereo.

Q: Gtx 100 has a lot of white noise when the gain, volume and master are all above 12 o'clock. Nothing but power plugged into it. Normal?

I haven’t had any issues like that. When playing with band it’s been quite.
