Q: is these offer include Preamplifier (SM93)?

I've used these for years. I don't believe that they contain a preamp.

Q: Can this mic be affixed to a performer's hairline?

Yes! We use these just below the actor's hairline, and they work perfectly! We create a hair rig with thin black elastic and toupee clips. The thin black elastic is threaded through the holes on the corners of the toupee clip in an X formation, and tied in a knot (having needle nose plyers helps with this). The mic is then threaded between the toupee clip and the elastic so that the mic cord isn't crimped by something like a bobby pin. I usually use 2-3 clips per mic cord. Then, the clips are clipped into the hair starting at the top of the head and going down. Good luck!

Q: How would this do for miking a violin? Anybody with experiencing using in that application?

Well, it's small enough and light enough for that, presuming you have a good point to attach it. There are microphones optimized for violins in terms of pickup distance and sound isolation to the wood, and it you wanted the best sound, I'd suggest one of those.
