Q: Are these normally worn with or without underwear?

I have worn underwear with them but they are lined so I don’t think you would have to.

Q: will the lining always stick out underneath the actual shorts?

Yes it will. I ended up sending mine back to get a size smaller. And I am going to have the lining hemmed up so it does not come down past the shorts. A lot of work but still worth it to be considering how comfortable they are.

Q: Zippered pockets on both sides?

Outside zippered pocket on the right back only. An zippered pocket on the side of the undershirts on the right side.
