Yes, the Alpha Harness is adjustable so that no matter your head size, you should get a comfortable, secure fit.

How long until i see results?

Varies with the individual, what your goals are, and where you currently rate relative to those goals. If you're new to neck training and your goal is to strengthen your neck, you'll see gains in as soon as a few weeks as your body gets more comfortable with the exercises and the muscles supporting the movement get stronger.

Does the harness come with a Chain or a nylon strap to use as a chain ?

The latest version comes with a reinforced nylon strap, whereas older versions came with a chain. If you order a new product from Iron Neck, it will come with a reinforced Nylon strap
