Can spray paint with this item?

No. The capacity is much too small. You could spray for about two minutes before the tank would be empty and unable to keep up with the output. No less than 26 gallon tanks should be used for painting. If you are detailing with a very low cfm then maybe you could get away with this unit.

Im a bit confused with the nail gages, for this particular model what size gage can it take other than 18?

That nail gun or “brad nailer” can only take 1 gauge, but you can get different length of nail, but it’s always 1 gauge.. You can get other hand tools that connect that take other gauges.

Does this item come with a hose or not? The description says it includes a hose. Some reviewers say it comes with a hose, others say it does not.

Yes it does come with a 15ft hose. It's listed in the package.

Has anyone used this compressor to spare paint rooms or decks, and if yes, did it work well for you?

IT was ok, just have to go slow and we used a smaller sized sprayer (Was only using it on a small deck so it wasn't an issue). All depends on your sprayer and how much you are going to be spraying at a time.
