Is this really three dollars for a one dollar juice package, and twenty dollars to ship it?

12 cartons. But keep in mind that this isn't mango juice. It's mango juice COCKTAIL. Most of this drink is water. Only 21% of it is mango juice. And there is added sugar.

How is it sealed? Does it have a screw top then a second pull tab inside?

It is a screw top. There's no additional pull tab. However, it's very watertight, sealed prior to and after opening. It's extremely tasty as well. As a personal note I sometimes add pineapple juice to mine which is a very nice mixture.
