It fits just as it looks. It is small but the slits on the outer thighs allow for ease of movement, however, they also allowed my butt to fall out the back. Surprisingly, my manhood was fairly secure and I didn't have the feeling that I was going to fall out, with normal use. Should I want to do back flips and cartwheels, maybe, but i'm not testing that. They are EXTREMELY comfortable. I"ll probably use them strictly for sleeping and to be worn with a drop-bottom onesie that i'm getting.

They are split and the split runs pretty high. These are supposed to be very tight fitting and ride high look at the picture. I have a 34 waist and could have gone with the medium but the large fits fine.
