Q: What is the distance between the ear hole and the bottom of the pillow?

The hole is not in the center. The hole is an oval 4 inches long and 3 1/2 inches wide. The hole is 4 1/2 inches from the bottom and 6 inches from the top. The entire distance from top to bottom is 14 1/2 inches (4 1/2 + 4 + 6 + = 14 1/2 inches).

Q: How should I wash and dry the pillow itself?

I washed mine in the washer. I even sprayed the soiled areas with spray and wash. Do not dry in dryer. Just set it up to dry. May take couple days. If uneven unzip and move inside around until even.

Q: Why isn’t this part of Prime!?

Don't know for sure, but it is made in England, so maybe accounting for their shipment fees to distributor.
