Does this come shipped with a cool pack?

No it does not. This is a dried product that comes in special packaging and is ready to use. I've been using this product for a few years with very good results.

I only need enough for 5 gallons. Can I measure out the appropriate amount and save the rest? How long does it keep?

I did similar, measured for 10 gallons, sealed the remainder in a ziplock and am storing in the freezer till next fall for another batch. It may last one year, but i am just assuming mlf bacteria behaves similarly to yeast. I wouldnt store for more than a year.

What is the typical amount of time does MLF go through?

It depends upon the temperature but I usually allow about six months while the wine itself settles down (clarifies) further.
