Yes, if this is purchased from The Daily Brick it will include the original tag.

No, I have used it everyday with my car keys for 3-4 months. It has not come apart or broken in any way. The paint on the helmet is slightly wearing off but the cape has not frayed.

Plastic, pretty much a well-glued lego. The chain is metal.
