I have cards that in a sealed plastic case that 4.5 inches in height. Does this mean they will only be secured by the bottom rail ?

there is no bottom rail. So, the end of the shelves are lipped. That is how the cards are prevented from sliding down flat.

Does it hold graded cards?

So, there are quite a few different type of graded card with different size plastic cases. Some are up to 7.25". And those won't fit. For this one, the height of the shelves are 5.75" ad the interior depth is 1.25.. You should measure whatever you and determine whether this would fit.

How many cards will this hold?

It will hold 35 PSA. BGS are a little wider so only 6 across will fit so 30. I think lime 3 BGS and 4 PSA might fit on a row to still get 3t but haven't tried it yet. Great case though.
