Q: does this kit include left and right side and gate and body piece for a total of four pieces?

Yes includes piece that bolts to truck and piece that bolts to tailgate for both sides. They where better then the originals. Install this set on my 2002 chevy Silverado 1500 ext. cab 4x4 still working great.

Q: Will this fit 2004 gmc 2500 HD crewcab driver side I bought two sets that did not fit the hole pattern and insert plug.

Not sure about a GMC. But it fit my Chevy 04 2500HD extended cab. I have a 6 ft bed. It was a direct fit
