Does this work with a Costco folding table?

It depends on the lip size of the edge of the table. I had to put a piece of wood under it for more stability onto the plastic table

There are 3 different sizes to secure the animal and they are adjustable.

Do I need a silicone Matt for the dog to stand on?

I use a folding table with the shelf liner (many makers, Duck, Gorilla, BC, can be found at Dollar store) for my 28 lb King Charles to stand on, NO more slipping. Best of Luck
