Can these be used manually with hands if you don’t have the machine?

I wouldn't think so. I don't think you could get the metal tight enough around the floral stem to for insertion.

Does it have a hole at top to stick in the twigs

I checked to make sure and there is no hole at the top. The tines on the sides could be bent inward to hold twigs or stems.
