Q: How does the extender attach to the bag? What is the width ?

This wraps around the bag's handle and attaches to itself with velcro. It is only about 3/4" wide, but that is a good width for gripping it.

Q: Would this work on a horizontal luggage handle?

Hello, Thank you for your question. The handle will attach to a duffel-style handle if that is what you're referring to. However it may make the bag harder to carry by adding extra length. This is typically recommended for the handles that extend from rolling luggage to allow for a little more reach as you wheel it around. No. It’s about as useful as tying a shoestring to any handle. I ended up throwing mine away.

Q: Will this attach to a store grocery cart.?

Im not sure how this would work on a grocery cart. This is not a stiff handle and is more meant to tow something. You could tow the shopping cart from the front but i don't see how it could be used while pushing it from behind.
