On dually motorhome tires how is it possible to reach in and hit the release button?

I had to remove my decorative hubcaps on round holes but otherwise it worked great if you have oval holes should work fine

How do i screw my milwaukee m18 inflator onto the 1/4" f npt fitting?

The Milwaukee M18 inflator does not appear to accept 1/4" NPT chucks - but rather has a standard screw-on nozzle. An adapter will need to be used for the unit to accept this chuck.

Why does the air keep flowing when not on a valve stem?

The chuck does not have a valve built in it. You have to connect the supply end to a valve, either a lever type or a pushbutton type valve.
