Would this be recommended for swelling?

I think it would work perfectly for minor swelling, but not if there is a significant amount of swelling, I think it would be to small.

The description says xs-xl. does this mean one size fits all? there is no place to specify a size, only a quantity.

I have mens 14, 4e (extra extra wide) feet and the velcro just barely connects around my foot but fits around my ankle just fine. You could always use something to wrap it or an extra piece of velcro, cut to whatever size you need. If you're smaller than me, you shouldn't have any problems.

Can you sleep in these?

Yes, however you shouldn't leave ice on any single area for more than 20 min at a time. Also, the ice pack will thaw after around 45 min. It is comfortable enough to sleep with it on in my opinion.

Ice packs will not fit in pockets, any suggestions?

Let them thaw a bit. Then fold or squeeze them and they will fit. Push all the way in then turn them. Takes some tweaking but they will fit. It really is a good ice pack.
