I punched a paper and then measured from the edge of the paper to the edge of the hole and it was 5/16 inch. Hope this is what you were looking for.

Can the punch heads be adjusted for paper that is 8.5" x 5.5"?

It looks certainly like if you loosen the screws and slide the punch heads along the slots, you can set the punch holes where you want them. And it looks anyway like you can remove say the center punch head or one or more of the others, completely, if you want to punch only 2 holes.

Would it work on 12" paper?

The three holes are centered for 11" . You can insert a 12" but you will have an extra inch at the top.
