Q: Why do some have a towel loop and some don't? With no clarification in description. The ones without a loop don't fit the same.

Q: Im 5'8" 29/30 waist 160pds athletic - would small be too small and or would medium be too big?

I'm 6'0, 180 with a 32 inch waist. I have the medium and they fit comfortably without tying the drawstrings. Small would probably fit you just fine.
