Q: Does it come with antenna? What gain?

Yes it comes with an antenna, once attached the lead extends about 30" and has a magnetic base that can placed horizontally/vertically. My speed went from 28+ to 70 mbps on average, currently on BT Superfast at 50 mbps. Upgrading to BT Plus so if the speeds are even faster, I'll update.

Q: do I need to connect the USB header?

The USB header is only for the Bluetooth functionality, so if you only want to use the wifi aspect of it you don't have to use the USB header.

Q: My order also didn't come with low profile brackets. In one of the questions you answered on the 16th Nov you said it does??

In one of the questions it says that it doesn't come with one. It doesn't.
